Meet the experienced and friendly doctors and staff who make Henryetta Eye Center the leading provider of vision care products and services in Henryetta.

Gary Jay LaValley, OD

Dr. Jay LaValley attendeed Oklahoma Christian University and the University of Oklahoma where he recieved his Bachelor of Buisness Administration. Then he began studies at Northeastern Sate University College of Optometry. He recieved his Doctorate in 1992 and was excepted in Beta Signa Kappa society of scholastic achievment. Dr. LaValley is married with 3 children.

Dr. LaValley purchased Dr. Cauthon’s practice in 1992 in Henryetta, OK. After 5 years, he and Dr. Michael Brian joined practices and formed The Henryetta Eye Center 1997. Dr. LaValley’s professional affiliations include the American Optometric Association and the Oklahoma Association of Optometric Physicians.

Among Dr. LaValley’s honors are Recognition of Outstanding Achievement- The University of Oklahoma 1986, Award of Clinical Excellence- NSUCO 1992, J. Harold Bailey Award – American Optometric Foundation 1992, President of the Henryetta Chamber of Commerce and Rotary International ( Henryetta Chapter).


Hayden Black, OD

Dr. Hayden Black was born and raised in Henryetta, OK. His interest in eye care began during his time as a patient at Henryetta Eye Center. After graduating from Henryetta in 2013, he pursued this interest at Northeastern State University where he earned his bachelor’s degree in Healthcare Administration. He then attended NSU’s College of Optometry, from which he received his Doctorate with honors in 2021. He was an active member of Beta Sigma Kappa, an optometric honor society reserved for students demonstrating high scholastic achievement.

Dr. Black’s professional affiliations include the American Optometric Association and the Oklahoma Association of Optometric Physicians. He is board certified in the use of therapeutic lasers to treat various eye conditions.

Dr. Black is committed to serving Henryetta and its surrounding communities. His goal is to provide each patient with the highest quality eye care possible.


Sarah Mulliniks, OD

Dr. Sarah Mulliniks was born and raised in Dewar, Oklahoma. She graduated from Dewar High School. She then attended Oklahoma State University where she received a bachelor’s degree in Cell and Molecular Biology and was a member of the Honor’s College. Dr. Mulliniks received her Doctorate of Optometry with honors from Northeastern State University Oklahoma College of Optometry in 2013 where she was also a member of the Beta Sigma Kappa Honor Society. Dr. Mulliniks is a member of the American Optometric Association and the Oklahoma Association of Optometric Physicians.
